Programs for Veterans and their Families
UMOM New Day Centers is a leader in providing services for homeless veterans and their families in Phoenix. Through the three grant awards from the
U.S. Department of Veteran Affairs we work with the Veterans Administration to provide housing, shelter and supportive services to these individuals and families who have sacrificed so much for our country.

Veteran Program Summary:
Transitional Living (Grant-Per-Diem for veterans and their families)
UMOM recently opened 8 units of Transitional Living on the main campus. Veterans are eligible if they have a referral from the VA Hospital, are able to live independently, and have dependent children. UMOM will case manage these families to help attain permanent housing within the community. The anticipated average length of stay is 6 months or less, but families may remain in the program for up to 2 years.

Supportive Services for Veteran Families (SSVF)
The SSVF program, located at UMOM New Day Centers and with a satellite program at Save the Family in the East Valley, assists very low-income veteran families. Due to limited financial resources, the SSVF program focuses on intensive case management to help participants increase their ability to sustain permanent housing and reach their greatest potential. Case managers will work with veteran families to identify VA benefits and other public benefits including, but not limited to: housing counseling services, health care services, child care services, educational assistance, legal services, transportation services, financial planning services, daily living services, and temporary financial assistance.
This program assists military families with housing needs and case management services. The program is stationed at UMOM New Day Centers with a satellite program at Save the Family in the East Valley. UMOM has hired five specialized staff to provide the following services: community outreach, case management, community resource and referral, and financial assistance for eviction prevention and rapid re-housing.
Permanent Supportive Housing
UMOM has been awarded this grant to provide ten veterans and their families with permanent supportive housing which includes rent subsidies, case management, and supportive services.
Eventually this program will be housed at Madison Pointe Apartments, which is an affordable housing community operated by The NRP Group that is expected to be completed in the summer of 2012. This community is located near the Phoenix VA hospital and will have a tenant preference established for military families. This program allows us to provide permanent housing, subsidy and support services to ten families within the Madison Pointe complex.
